California residents have the right to request that Optio Solutions disclose certain information to you about the collection and use of your personal information over the past twelve (12) months.  Under the California Consumer Privacy Act (CCPA), you have the right to request any or all of the following personal information:

• The categories of personal information collected about you.
• The categories of sources from which the personal information is collected.
• The business or commercial purpose for collecting or selling the personal information.
• The categories of third parties with whom we share personal information for a business purpose.
• The specific pieces of personal information collected about you.

You also have the right to request that we delete any of your personal information that we collected from you and retained, subject to certain exceptions.  We do not sell your personal information except in connection with an activity subject to the Fair Credit Reporting Act, which is exempt under the CCPA. We will respond to a verifiable consumer request within forty-five (45) days of receiving it. If additional time is required, we will inform you of the reason and extension period (up to an additional 45 days), in writing.  If you have an account with us, we will deliver our written response to that account.  If you do not have an account with us, we will deliver our written response by mail or electronically, at your option.  Any disclosures we provide will cover the twelve (12) month period preceding receipt of the verifiable request.  Our response will also explain the reason(s) we cannot comply with a request, if applicable.  We will use the information you provide on this form only for the purpose of verifying your identity and the personal information you are requesting to know or delete, to respond to your request and to keep a record of your request and our response.
